Cancellation fees are 20% of the total cost of the package if canceled within one week of the scheduled drop off date. If training is rescheduled after original dates are set Collins Canine cannot guarantee availability of either boarding or training. Rescheduling fees may also apply to changes made within one week of the originally scheduled drop off date. I, as the owner/handler or owner's representative of the dog(s) listed above do hereby release and hold harmless, Collins Canine and all their training staff officers, agents, associates and all persons connected therewith, from any and all liability, claims or causes of action which may arise from any and all participation in dog training or other related activities performed by the above referenced individual(s).
